The best way to schedule an appointment with me is to contact me via email by sending me a (legible) copy of your insurance card front and back along with your driver license, and a contact number and based on my availability.  I will schedule you my first available appointment. Once I receive all this information and verify your insurance I will contact you back within 24 hours business days with my first available appointment. I will also be sending you a copy of my Welcome Letter and Suboxone Contract for your review and agreement.
